Feder: there are 40 newly qualified citizens to deal with diabetes and community expectations

"Active and competent citizen", the first edition

The whole training course was divided into a series of lessons, for a total of 70 hours during 2018. 40 delegates from all the diabetic patient associations present in the Emilia-Romagna area took part; as teachers, experts and professionals from the regional health service participate.

The objective of the course, to contribute to supporting patient associations on their ability to take advantage of the challenge of active participation and effective dialogue with all actors of the "health system", in accordance with what the Scheme describes and predicts Health and Health 2017/2019, from the National Plan for Diabetic Disease and the National Chronic Plan.

The interactive methodology has provided plenary sessions on general topics, favoring comparison and deepening. Information on regional routes and projects has created new opportunities to take action to tackle exclusion and vulnerability, for the co-construction of pre-care life projects, particularly in the context of health homes, a referral point. to aim for citizens to access primary care.

Diabetes and integrated disease management in Emilia-Romagna: the model works
